This monitor was bought by HR as a disability 
accomadation, please talk with Laurie about 
exactly what monitor configuration she wants to 
end up with(dual, single, etc.)  Invoice for the 
monitor will be going straight to Human Resources 
so have Laurie Burton/ office staff in MNB sign 
the NEW EQUP Form on the bottom to acknowledge 
they REC'd it
