Immunization Requirements

    POLICY

    Following recommendations from the American College Health Association (ACHA), 
    The American Academy of Pediatrics (AAP), The immunization Practices Advisory 
    Committee (ACIP), and the Oregon Health Division (OHD), incoming students (those 
    taking 12 or more undergraduate hours or 9 or more graduate hours) will be 
    required to provide proof of immunization for the following:
    
    1.   Tetanus and Diphtheria - primary series plus a booster within the past 
         10 years.
    
    2.   Rubeola (Red measles) - students born Jan. 1, 1957 or later must show proof 
         of two doses of measles vaccine, the first dose after 12 months of age.
    
    3.   Rubella (German measles) - students born Jan. 1, 1957 or later must show 
         proof of one immunization after 12 months of age.
    
    RECOMMENDED:
    
    1.   Tuberculosis - A tuberculin skin test (either PPD or Tine) within 12
         months of admission.  Students who show a positive reaction to the skin 
         test must have a chest x-ray.  A written record of the date of the x-ray 
         results and physician's signature is required.  Students who have a past 
         history of a positive skin test or tuberculosis must have a chest x-ray 
         within 12 months of admission.  Written documentation is required, 
         including date and results of documentation is required, including date 
         and results of positive skin test, treatment, and medications taken.
    
    Students may be exempt from meeting the requirements based on religious beliefs,
    or with a physician's documentation of disease, positive titer or 'that the 
    vaccine poses a health risk.'  During a measles outbreak, students who have a 
    medical or religious exemption will be excluded from school for 21 days from 
    exposure date.
    
    A grace period of one term is granted to complete these requirements.  If proof 
    of two doses of Rubeola vaccine is not provided by the end of the term, 
    registration for the next term, registration for the next term will be held 
    until the student complies with the regulations.
    
    

    PURPOSE

    
    
    

    PROCEDURES

    Each entering student is required to provide a completed Health History form to 
    the Student Health Center which includes the dates of required immunizations.  
    If the immunizations do not meet compliance, notifications will be sent to the 
    student.
    
    The Registrar's Office will provide the Student Health Center with a list each 
    term of new incoming students.  The Student Health Center will notify the 
    Registrar of all students in compliance.  A hold will be placed on the record of 
    each student not in compliance and payment of fees will be denied that student 
    for the next term.  This hold will be removed by the Student Health Center only 
    when the student provides documentation of immunizations or exemption 
    statements. 
    
    

    AUTHORITY

    This policy is based on Oregon Administrative Rule 580-10-005 states that, "All 
    students must present appropriate proof of immunizations and tests required by 
    policies established by institutional presidents."
    
    

    APPROVAL

    Student Health Center and Vice President for Student Affairs and Enrollment 
    Management.
