FULL STATEMENT OF AUTHORITY | |
POLICY STATEMENT This policy outlines the actions mandated for university personnel for situations involving a student's threat of harm to self, threat of harm to others, and attempted suicide.
PROCEDURES I. Any person or department receiving a call or threat will contact Campus Public Safety at 503-838-8481 or 503-932-3419 (after hours). A. Campus Public Safety Officers will respond and make initial assessment. B. Campus Public Safety Officers will notify the Director of Student Health and Counseling Center or designee, the Vice President for Student Affairs or designee, and the Director of Campus Public Safety or designee. C. All "in progress" or suicide attempts should be reported to Campus Public Safety by calling the emergency number, 503-838-9000.
II. On-Campus Resident/Day (Serious threat of harm to self or others) A. Assessment will be made by Campus Public Safety Officers who will then decide whether to dispatch off-campus emergency responders. B. Student Health and Counseling Center will be notified by Campus Public Safety. Records from all involved departments will be available to the Director of Student Health and Counseling Center in order to facilitate information exchange with "need to know". C. Vice President for Student Affairs will be notified by Campus Public Safety. Information will be exchanged by all parties in the best interest of the student. D. If initial assessment is not life-threatening as determined by emergency responders, Campus Public Safety will take the student to Student Health and Counseling Center for evaluation. E. If the student refuses to go to the Student Health and Counseling Center, Campus Public Safety will inform the Vice President for Student Affairs.
III. On-Campus Resident/Night, Weekend (Serious threat of harm to self or others) A. Any person who receives information regarding a possible suicide during the evening or during a weekend should call Campus Public Safety 503-932-3419. B. The Campus Public Safety Officer who responds will make an initial assessment. The Campus Public Safety Officer will then notify the on-call counselor of Student Health and Counseling Center. The officer will also notify the Vice President for Student Affairs and the Director of University Housing.
IV. Off-Campus Student/Day, Night, Weekend (Serious threat of harm to self or others) A. Initial notification from outside agencies/sources regardless of who is initially contacted - Vice President for Student Affairs, Campus Public Safety, and Student Health and Counseling Center will be notified and will coordinate response. B. Information will be exchanged from Student Health and Counseling Center in the best interest of the student. C. Follow-up will be confidential through Student Health and Counseling Center and recommendations for the student's continuation on campus or in classes will be coordinated by Director or designee.
V. Completed Suicide A. Initial notification from outside agencies/sources regardless of who is initially contacted - Vice President for Student Affairs, Campus Public Safety, and Student Health and Counseling Center will be notified by the first department that gains knowledge and will coordinate response. B. Death of a Student policy will be followed.
VI. Actual Harm to Another A. Campus Public Safety, upon notification, will take appropriate action and advise Vice President for Student Affairs and Director of Student Health and Counseling Center.
VII. Suicide Thoughts Without Plan A. Student Health and Counseling Center will be notified and will evaluate the situation by phone or in person. B. In-House Procedure - report will be made to Student Health and Counseling Center and Vice President for Student Affairs.
VIII. Overview De-briefing will be held within 48 hours of all events, including Vice President for Student Affairs, Campus Public Safety, University Housing, Student Health and Counseling Center, and any other involved departments. | |
