The form can be found in your portal.

So the first step is to log into your portal.

Once you've logged in scroll about half-way down the form

and you'll see the 20-21 financial aid forms.

Look for the degree partnership program change homeschool form

which is fourth on the list.

Click on that and this will bring the form up for you.

As you can see, it'll auto populate your V#, first and last name.

It then asks you for a little bit more information.

The first is who do you want your homeschool to be.

For this example, I'm going to select Chemeketa.

Then it asks for you student ID at your community college.

Since I don't actually have a K# from Chemeketa

I'm going to make one up.

It then asks you to select the term that you

want to change your homeschool for. I'm going to select fall.

Read through the rest of the document and once you've read it

click on I agree and then click submit.

Once it's been submitted it will go to our financial aid office and

they will process your form.  They process the forms every Monday.

And once your form has been processed you will receive

an email from our financial aid office.

In general,we do not recommend changing your homeschool during the
